chained select only works once

I'm trying to get the chained select script to work with my site. I've got it installed per the instructions and it works....the first time at least. Once I've selected something once with the first select box, the second box populates like it should. But if I try to change the first select, then then second select goes empty and will not repopulate -- even if I go back to my first selection. My console shows that only one POST requests has been sent, so it looks like the script isn't getting anything from the second select change to go find.

I've tried this in FF and IE7 and it did the same thing in both. Here is where my test page is located: .
