Actually, I put them all into a function with a FOR loop, just increment the name of the poll. It's all one form, because you've only got one button, so it remains form[0] throughout.
I did a similar script to get the cookie to set all the polls for one-time voting.
I just have a button refresh the page to clear everything so people who took a peak first can vote. There's probably a better way to do this, but it works.
Pretty cool; I've got five polls running simultaneously. Fun to watch them all go together.
Keep plugging away and you figure it out. This is a nice script, but the documentation and backup don't quite match up. Whatever doesn't kill you...

This is a nice script. But, because I'm using it in a small space (125px X 125px), with 1 question, 3 possible answers, I decided to randomize the poll
Having a lengthier poll on a page with more space would be nice, mind sharing your code?
I hear ya on the documentation. There's another script I found (rating, not poll), really nice script but its documentation (and lack of a forum) make this look like a class room.
